WHO WE SERVE

Is Placement Support Right for You?

Our placement services are designed for patients, families, and caregivers navigating one of life's most important decisions.

Aging Adults & Seniors

Patients who need a higher level of care than can be provided at home whether due to functional decline, chronic illness, or safety concerns. We assess your medical needs and match you with the right care setting.

Families & Caregivers

Family members who are overwhelmed, unsure where to start, or trying to find the right fit for a loved one. We guide you through the process, explain your options, and coordinate with facilities on your behalf.

Hospital Discharge Cases

Patients being discharged from the hospital who need placement in a skilled nursing facility, assisted living, or family care home before they can safely return to independent living. We handle the documentation and coordination.

THE PROCESS

How Placement Works with Dr. Lucas

A clear, step-by-step process from initial consultation to placement with medical guidance at every stage.

Consultation

We start with a conversation in office, by phone, or via telehealth to understand the patient's medical history, current needs, and what the family is looking for in a care setting.

Evaluation

Dr. Lucas conducts a thorough clinical evaluation, including functional status and capacity assessments, to determine the appropriate level of care and complete required documentation including FL-2 forms.

Coordination

We review placement options based on medical needs, coordinate with social workers and case managers, and communicate directly with assisted living facilities, family care homes, and other providers.

Coordination

After placement, Dr. Lucas remains available for follow-up medical care, ensuring continuity and a smooth transition into the new care environment — whether in person or via telehealth.

COMMON QUESTIONS

Placement FAQs

What is an FL-2 form and why is it needed?

An FL-2 is a North Carolina standardized form used to document a patient's medical status, functional abilities, and level of care needs. Most assisted living facilities and family care homes require a completed FL-2 before admission. Dr. Lucas is qualified to complete and certify FL-2 evaluations as part of our placement services.

How do I know what level of care my loved one needs?

That's exactly what our evaluation is for. Dr. Lucas will assess your loved one's medical history, current functional status, cognitive abilities, and daily living needs to recommend the most appropriate type of care setting. You don't need to figure this out on your own.

Can you help us find specific facilities in the Triad?

Yes. We coordinate directly with assisted living facilities, family care homes, and memory care providers in the Piedmont Triad area. We can help review placement options based on your loved one's medical needs, preferences, and available resources, and help facilitate communication with those facilities.

Does insurance cover placement services?

Coverage varies by plan. Medical evaluations such as the clinical assessment and FL-2 completion may be covered under your insurance. Consultation and coordination services may be billed differently. Call our office and we'll help clarify what your plan may cover before your appointment.

What if my loved one is being discharged from the hospital soon?

Contact us as soon as possible. Hospital discharge placements are time-sensitive and we understand the urgency. Dr. Lucas can often conduct an evaluation and complete required documentation quickly to help facilitate a smooth transition from hospital to the appropriate care setting.

Do you continue care after placement?

Yes. Dr. Lucas provides follow-up medical care after placement either through office visits, house calls to the care facility, or telehealth visits when appropriate. We don't disappear once placement is complete.
