An FL-2 is a North Carolina standardized form used to document a patient's medical status, functional abilities, and level of care needs. Most assisted living facilities and family care homes require a completed FL-2 before admission. Dr. Lucas is qualified to complete and certify FL-2 evaluations as part of our placement services.
That's exactly what our evaluation is for. Dr. Lucas will assess your loved one's medical history, current functional status, cognitive abilities, and daily living needs to recommend the most appropriate type of care setting. You don't need to figure this out on your own.
Yes. We coordinate directly with assisted living facilities, family care homes, and memory care providers in the Piedmont Triad area. We can help review placement options based on your loved one's medical needs, preferences, and available resources, and help facilitate communication with those facilities.
Coverage varies by plan. Medical evaluations such as the clinical assessment and FL-2 completion may be covered under your insurance. Consultation and coordination services may be billed differently. Call our office and we'll help clarify what your plan may cover before your appointment.
Contact us as soon as possible. Hospital discharge placements are time-sensitive and we understand the urgency. Dr. Lucas can often conduct an evaluation and complete required documentation quickly to help facilitate a smooth transition from hospital to the appropriate care setting.
Yes. Dr. Lucas provides follow-up medical care after placement either through office visits, house calls to the care facility, or telehealth visits when appropriate. We don't disappear once placement is complete.
